Job Opportunity: Registered Nurse - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Summary
As a Home Health Registered Nurse, you will:
- Assess patient conditions in their homes
- Implement individualized care plans
- Provide ongoing education and support to patients and their families
- Perform admission processes and case management duties
- Document thoroughly and communicate effectively within an interdisciplinary team
- Work weekend rotations approximately once every 6 weeks
- Travel to patient cases as assigned
Required Qualifications
- Valid Registered Nurse (RN) license in Massachusetts
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum 1 year of experience in Home Health within the last year
- Minimum 1 year of experience in a Home Care setting
- Minimum 1 year of Case Management experience
Health and Compliance Requirements
- Documentation of immunizations or immunity for MMR (measles, mumps, rubella)
- TB screening within 3 months of start date (skin test or blood test) and updated if there is a break in service longer than 6 months
- Hepatitis B vaccination or proof of immunity is strongly preferred
- Tdap vaccination within the last 10 years is preferred
- Physician health statement within 1 year of start date
- N95 respirator fit test within 1 year of start date and updated annually (no exemptions allowed)
- Respirator medical clearance attestation if fit test is required
- Criminal background checks including national sex offender registry, CORI (Massachusetts), and exclusion lists (OIG, GSA, Medicaid) within 45 days prior to start and updated as required
- Motor Vehicle Record (MVR) check within 45 days prior to start (for all home care positions)
- Nurse Aide Registry check within 30 days of start (required by MA Department of Public Health for all employees)
Submission Requirements
- Resume including past 7 years of work experience with city and state
- Two professional references within the last 4 years (at least one supervisor or higher)
- Skills checklist completed within 1 year
- Candidate submission coversheet (current version)
- Confirmation of assignment form upon acceptance and for any extensions
- Name change documentation if legal name differs from credential documentation
Additional Information
- A professional photo (color, plain background, no filters or glasses) taken within the last 6 months is required for onboarding
- Background checks and verifications are conducted through designated processes and must be clear of disqualifying issues
- Weekend rotations occur approximately once every 6 weeks
This position is ideal for experienced Home Health RNs who are comfortable with travel and case management responsibilities in a community health setting.
