Job Opportunity: Case Manager (Registered Nurse)

Location: Smithfield, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

Home health case management experience required
Proficiency with OASIS (Outcome and Assessment Information Set) required
Minimum 1 year of nursing experience in specialty required
Local candidates allowed at same rate
Day shift: 5 days per week, 8 hours per day (8:00 AM – 4:30 PM)

Required Qualifications

Valid Registered Nurse (RN) license in Virginia
Minimum 1 year of case management experience within the last year
Experience in a home health setting
Proficiency with OASIS documentation
BLS certification issued by the American Heart Association or American Red Cross
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
