$250 CONTRACT COMPLETION BONUS
Opportunity:
RN - Case Management
Job Details:
• Work first shift Monday through Friday from 8:00 AM to approximately 4:30 PM
• No weekend rotation, on-call, or holiday requirements
• Minimum 2 years of nursing experience required
• Travel experience preferred over first-time travelers
• Manage caseloads of 14-16 patients per RN
• Provide care for end-stage hospice patients
• Work in various settings including patient homes, assisted living facilities, skilled nursing facilities, group homes, independent living, and 2 hospitals for general inpatient care
• Travel within the community to patient homes for care delivery
• Required certifications: BLS, valid driver's license, and auto insurance
• Work with EMR systems Epic and Rover
• Utilize IV pumps, medication dispensing systems, chair and bed alarms
• Manage home health supplies including bag and trunk supplies
• One week facility and unit orientation provided
• Scrub color requirements: blue or black
• Free parking available at Burlington and Walworth offices
• Unit culture emphasizes teamwork and team support
