Opportunity:

RN - Case Management

Job Details:

• Work Monday through Friday from 8:00 AM to 4:30 PM
• Manage 13-15 patient caseload providing hospice care
• Visit patients in home, assisted living facilities, and skilled nursing facilities
• One weekend rotation per month required
• One call shift per month required
• No holiday expectations
• Orientation up to 1 week based on comfort level
• Float anywhere within assigned counties as needed
• Maintain 1:1 patient ratios during visits
• Provide end of life care and planning
• Conduct wound assessments and care coordination
• Manage pain and symptom management for patients
• Handle patient triage responsibilities
• 2 years of hospice experience required
• Previous admission experience required
• EPIC Remote Client experience required
• Required certifications: BLS, valid driver's license and insurance
• Work with adult end-of-life patients only
• Use EPIC Remote Client EMR system and Sapphire IV pumps
• Dress code allows any color scrubs or business casual attire
