Estimated Gross Weekly Pay: $3480.00
$250 CONTRACT COMPLETION BONUS
Opportunity:
RN - Case Management
Job Details:
• Work Monday through Friday from 8:00 AM to 4:30 PM
• Four 10-hour shifts may also be available
• No weekend rotation required
• No call responsibilities
• No holiday expectations
• Provide end of life care and planning for adult patients only
• Perform wound assessments and care coordination
• Manage pain and symptom management
• Handle triage responsibilities
• Maintain 1:1 patient ratios
• Float anywhere within the counties as needed
• Orientation up to 1 week based on comfort level
• Required certifications: BLS, valid driver's license and insurance
• 2 years of hospice experience required
• EPIC Remote Client experience preferred
• Work with IV pumps (Sapphire)
• Dress code: any color scrubs or business casual
• Manager approval required for all time off requests
