Key Responsibilities

• Perform nursing assessments on all patients within 8 hours of admission and develop individualized plans of care in collaboration with the patient, family, and interdisciplinary team.
• Provide accurate, timely documentation of all assessments, interventions, and patient responses in the medical record in accordance with policy.
• Participate in patient rehabilitation programs, utilizing the nursing process of assessing, planning, implementing, and evaluating patient care with an emphasis on patient and family education.
• Initiate discharge planning from the day of admission and communicate pertinent changes in patient status to attending physicians, charge nurses, and other care providers to ensure continuity of care.
• Provide direction, training, and supervision of Licensed Practical Nurses, Certified Nursing Assistants, and orientees through effective delegation and problem-solving.
• Assume charge nurse responsibilities following orientation period as requested and contribute to a positive, safe, and efficient unit environment.

Required Qualifications

• Current, valid Registered Nurse (RN) licensure in the state of Iowa.
• Demonstrated ability to perform nursing assessments and develop individualized care plans.
• Proficiency in accurate and timely clinical documentation within a medical record system.
• Ability to supervise and delegate to ancillary nursing personnel including LPNs and CNAs.
• Strong commitment to patient safety, infection control, and adherence to hospital policies and procedures.

Preferred Experience / Skills

• Prior experience in a rehabilitation or long-term acute care setting.
• Experience participating in interdisciplinary team conferences and discharge planning processes.
• Demonstrated commitment to continuing education and ongoing professional development.
