Contract | 13-Week Assignment | Days | Monday – Friday, 8:00 AM – 5:00 PM | Onsite

Key Responsibilities

Provide occupational therapy services to patients in a home health setting across an assigned territory
Conduct evaluations and develop individualized treatment plans to support patient functional goals
Deliver skilled OT interventions focused on improving daily living activities, safety, and independence
Document patient progress and clinical notes using HCHB (HomeCare HomeBase) EMR
Coordinate care with interdisciplinary team members to ensure continuity of patient care

Required Qualifications

Active Occupational Therapist (OT) license in the state of Washington
Minimum of 12 months of Home Health experience within the past 3 years
Proficiency or ability to work within HCHB (HomeCare HomeBase) EMR
Valid driver's license and reliable transportation for home visits within assigned territory

Preferred Experience / Skills

Familiarity with Spokane County and surrounding areas
Experience managing a caseload across a multi-county territory
Strong time management and independent practice skills in a field-based setting
