Key Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Design functional treatment programs that support effective return to community activities, progressing patients toward discharge within an established length of stay
• Provide delegation and direction of all discipline-specific therapy services for assigned patients
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education

Required Qualifications

• Active Physical Therapist license in good standing in the state of Florida
• Ability to perform 60-minute evaluations and treatments in an outpatient setting
• Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards
• Commitment to patient and employee safety practices, including safe patient handling and infection control protocols

Preferred Experience / Skills

• Experience or coursework in outpatient physical therapy settings
• Familiarity with functional quality indicator assessments and documentation in electronic medical record systems
• Demonstrated interest in professional development through continuing education and involvement in professional organizations
