As a Hospice RN Case Manager, you will admit patients to service and assess their physical, psychosocial, and safety needs. Your role involves developing individualized care plans, coordinating with a multidisciplinary team, and conducting follow-up visits. This position is vital in ensuring quality care in the hospice setting, with a focus on the patient’s environment and ongoing assessments.
