RN – Case Manager (13 Weeks, Dayshift)
Schedule: Monday–Friday, 8:00 AM–5:00 PM (30‑minute lunch) Call: None Location: St. David’s Round Rock Medical Center – Ground Floor Department: Case Management
Unit Overview
- Hospital Size: 209 beds
- Team Size: 27 total Case Management staff
- Staffing Model:
- Days: 7 RN Case Managers, 5 Social Workers, 1 Case Management Assistant
- Nights: 1 Social Worker (ED coverage only)
- Patient Ratios:
- RN Case Manager: 1:25
- Social Worker: 1:40
- Equipment: Workstations on Wheels (WOWs)
- EMR Systems: Midas, Meditech, Interqual, CarePort
- Permanent roles available: Yes
Role Summary
The RN Case Manager supports care coordination and discharge planning across a diverse inpatient population. This role partners closely with Social Workers, physicians, nursing staff, and ancillary teams to ensure safe, efficient transitions of care.
Required Qualifications
Candidates must have the following to be considered:
- Prior travel nursing experience
- Case Management experience in an acute care setting
Preferred Qualifications
Candidates with these skills will be prioritized:
- Experience within HCA Case Management
- Proficiency with Midas, Interqual, and Meditech
Core Responsibilities
- Care coordination across the continuum
- Discharge planning and facilitation
- Collaboration with interdisciplinary teams
- Documentation within multiple EMR platforms
