📍 Assignment Details

• Location: Grand Junction, CO
• Duration: 13 weeks
• Shift: Monday–Friday | 8:00 AM – 5:00 PM

🩺 Role Overview

As a Primary Hospice RN Case Manager, you will:

• Manage and coordinate care for an assigned patient caseload
• Lead the interdisciplinary care team for each patient
• Provide symptom management and end-of-life care
• Educate and support patients and caregivers
• Help families define and achieve personal care goals
• Deliver care across home, nursing, and assisted living settings 

✅ Requirements

• Active RN license
• Minimum 2 years of nursing experience
• Strong clinical judgment, assessment, and critical thinking skills
• Ability to manage multiple patients and priorities independently 

📜 Certifications

• BLS / CPR – required

• Strong communication and documentation
