Travel RN – Hospice Case Manager | M–F Days | Meaningful, Patient-Focused Care

Job Overview

Specialty: RN Case Manager (Hospice)
Schedule: 5x8s (Day Shift)
Shift: 7:00 AM – 3:00 PM (Monday–Friday)
Contract Length: 13 weeks
Start Date: ASAP
Guaranteed Hours: 40 per week
Setting: Hospice / Home-Based Care

Responsibilities

Manage a caseload of hospice patients across home and care settings
Perform patient assessments and develop individualized care plans
Coordinate care with interdisciplinary teams
Provide symptom management and end-of-life care
Educate patients, families, and caregivers
Support emotional and physical needs of patients and families

Requirements

Active RN license
BLS certification required
Case management and/or hospice experience preferred
Strong assessment and critical thinking skills
Comfortable working independently in the field

Additional Details

Monday–Friday schedule (great work-life balance)
Occasional on-call or weekend backup
Mix of home, assisted living, and facility-based care
Focus on compassionate, patient-centered care

Hospice or case management experience preferred. Strong communication and patient-centered care skills required. Comfortable managing a caseload independently.
