Shift: 8 Hour Days
Schedule: Day Shift + Every Other Weekend (EOW)
Guaranteed Hours: 40 hours/week
Contract Length: 13 Weeks

Job Summary:

We are seeking an experienced Registered Nurse (RN) for a Case Manager role within an acute care setting in California. This position focuses on coordinating patient care, discharge planning, and ensuring continuity of care across the healthcare system.

⚠️ Important: Candidates must have prior experience with Dignity Health or CommonSpirit Health systems to be considered.

Key Responsibilities:

• Coordinate patient care plans and discharge planning
• Collaborate with interdisciplinary teams
• Ensure continuity of care across services and facilities
• Advocate for patient needs and appropriate resource utilization
• Maintain accurate documentation in EMR

Requirements:

• Active California RN License (required at submission)
• BLS Certification (required)
• LA City Fire & Safety Card (required)
• Prior Dignity Health / CommonSpirit experience – REQUIRED
• Strong Case Management experience
• Acute care background required

Schedule & Work Expectations:

• Shift: 8-hour days
• Weekend Requirement: Every other weekend
• Floating: Yes (First to float)
• Guaranteed Hours: 40 hours/week
• Facility may cancel up to 4 shifts per 13-week assignment

• Charting System: Cerner
• Setting: Acute Care Hospital
