Job Summary:

We are seeking an experienced RN Case Manager to support care coordination and discharge planning in a fast-paced acute care hospital setting. The ideal candidate will have strong clinical judgment, excellent communication skills, and experience managing high patient volumes.

Key Responsibilities:

• Perform care coordination and discharge planning for assigned patients
• Conduct needs assessments and develop plan of care
• Ensure appropriate utilization review and medical necessity
• Coordinate with interdisciplinary teams, patients, and families
• Arrange post-discharge services, DME, and follow-ups
• Handle prior authorizations and benefits eligibility
• Maintain accurate documentation in Epic EMR
• Ensure compliance with CMS, HIPAA, and regulatory guidelines
• Manage a patient ratio of approximately 1:20
• Float between units as needed

Requirements (Must Have):

• Minimum 2 years of acute hospital Case Management experience
• Strong experience in Care Coordination & Discharge Planning
• Experience working in a fast-paced hospital environment
• Epic EMR experience
• BLS Certification
• Travel experience (preferred but can screen if experience is strong)

Preferred Qualifications:

• Experience in teaching, community hospital, or LTAC settings
• Familiarity with InterQual criteria and utilization review
• Experience across specialties (ICU, ED, Med-Surg, etc.)
• Knowledge of regulatory standards (CMS, Joint Commission, DRG, ICD-10)

Work Details:

• Weekends: 2 weekends every 6 weeks
• On-call: Not required
• Floating: Yes, based on unit needs
