Description
Job Summary:
We are seeking an experienced RN Case Manager to support care coordination and discharge planning in a fast-paced acute care hospital setting. The ideal candidate will have strong clinical judgment, excellent communication skills, and experience managing high patient volumes.
Key Responsibilities:
- Perform care coordination and discharge planning for assigned patients
- Conduct needs assessments and develop plan of care
- Ensure appropriate utilization review and medical necessity
- Coordinate with interdisciplinary teams, patients, and families
- Arrange post-discharge services, DME, and follow-ups
- Handle prior authorizations and benefits eligibility
- Maintain accurate documentation in Epic EMR
- Ensure compliance with CMS, HIPAA, and regulatory guidelines
- Manage a patient ratio of approximately 1:20
- Float between units as needed
Requirements (Must Have):
- Minimum 2 years of acute hospital Case Management experience
- Strong experience in Care Coordination & Discharge Planning
- Experience working in a fast-paced hospital environment
- Epic EMR experience
- BLS Certification
- Travel experience (preferred but can screen if experience is strong)
Preferred Qualifications:
- Experience in teaching, community hospital, or LTAC settings
- Familiarity with InterQual criteria and utilization review
- Experience across specialties (ICU, ED, Med-Surg, etc.)
- Knowledge of regulatory standards (CMS, Joint Commission, DRG, ICD-10)
Work Details:
- Weekends: 2 weekends every 6 weeks
- On-call: Not required
- Floating: Yes, based on unit needs
- Locals accepted: Yes
