Description
Job Title: RN Case Manager (Discharge Nurse)
Location: Los Angeles, CA
Duration: 13 week
Shift: Day 5x8hr (11a-730pm M-F- 40hrs a week)
Job Description:
- Plan and coordinate patient discharge processes to ensure safe transitions of care
- Assess patient needs and identify barriers to discharge
- Collaborate with physicians, nurses, case managers, social workers, and other healthcare professionals
- Educate patients and families regarding medications, discharge instructions, follow-up appointments, and home care needs
- Arrange post-discharge services such as home health, rehabilitation, skilled nursing, or medical equipment
- Review and reconcile medications prior to discharge
- Ensure all discharge documentation is completed accurately in the EMR system
- Communicate discharge plans with patients, families, and healthcare teams
