Description

Position Details

Setting: Skilled Nursing Facility (SNF)
Location: Jamestown, Virginia
Schedule: Full-Time | Monday–Friday
Patient Population: Primarily geriatric and post-acute rehab patients
Start Date: ASAP

Responsibilities

Evaluate and treat patients with orthopedic, neurological, and medically complex conditions
Create individualized treatment plans focused on mobility, strength, balance, and functional independence
Collaborate with interdisciplinary rehab and nursing teams
Complete daily EMR documentation and progress notes
Educate patients and families on treatment plans and discharge recommendations
Maintain productivity standards while delivering quality patient care

Qualifications

Graduate of an accredited Physical Therapy program
Active Virginia PT license or ability to obtain one
SNF or geriatric experience preferred
Strong communication and teamwork skills
New grads welcome depending on facility support and mentorship availability
