Location: Solon (Cleveland), Ohio

Position Overview
A leading home health provider in the Cleveland market is seeking an experienced Physical Therapist (PT) for a 13-week travel assignment. This role offers the opportunity to work independently while providing high-quality, patient-centered care in patients' homes throughout the greater Cleveland area. The therapist will evaluate patients, develop individualized treatment plans, coordinate care with interdisciplinary teams, and help patients maximize safety and independence in their home environment.
Responsibilities
- Complete initial evaluations and ongoing reassessments
- Develop and implement individualized treatment plans
- Provide skilled physical therapy interventions in the home setting
- Collaborate with physicians, nurses, therapists, and caregivers
- Complete timely and accurate documentation
- Educate patients and families on home exercise programs and safety
- Participate in care planning and discharge planning activities
Requirements
- Active Ohio Physical Therapist License
- Current BLS Certification
- Minimum of 1 year of recent PT experience preferred
- Home Health experience strongly preferred
- OASIS experience preferred
- Valid driver's license and reliable transportation
