SNF PT Opportunity – Lake Worth, FL

Ardor Health Solutions is seeking a Physical Therapist for a Skilled Nursing Facility travel assignment in Lake Worth, Florida.

This is a great opportunity for a PT who enjoys working with an adult and geriatric rehab population. You'll evaluate and treat patients recovering from hospitalization, surgery, illness, or injury, with a focus on improving strength, balance, mobility, safety, and functional independence.

• Gait, balance, strengthening, and mobility training
• Collaborative interdisciplinary rehab team

Qualifications Required

• Graduate of an accredited Physical Therapy program
• Current Florida PT license or ability to obtain prior to start
• Current BLS certification
• Strong evaluation and treatment skills
• Strong documentation and communication skills
• Comfortable working with adult and geriatric patients
• SNF or geriatric rehab experience preferred

What You'll Do

• Complete PT evaluations and reassessments
• Develop individualized plans of care
• Provide gait, balance, strengthening, transfer, and mobility training
• Help patients improve functional mobility and independence
• Provide fall-prevention and safety education
• Educate patients and caregivers
• Collaborate with PTA, OT, COTA, SLP, nursing, and other members of the care team
• Assist with discharge planning and recommendations
• Complete timely clinical documentation
