Required Qualifications

• Valid RN license in New York
• Med Surg / Telemetry (3 years of experience)
• Med Surg / Telemetry experience within the last year
• BLS issued by American Heart Association or American Red Cross
• ACLS issued by American Heart Association or American Red Cross
• NIHSS
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License
• Ability to read basic rhythm strips
• NIH Stroke Scale assessment capability

Onboarding / Compliance

• Influenza vaccine or facility-specific declination form required; if declination is used, traveler must wear a mask at all times while on assignment
• MMR immunity documentation required
• Varicella immunity documentation required
• Hepatitis B immunity documentation or declination required
• Tdap within the last 10 years, or facility-specific declination form if not on file
• TB testing required
• Fit test must be completed prior to start and within the last 12 months
• Physical/health statement within the past 12 months
• Driver’s license or state ID required
• Agency background check required
• Agency drug screen required
• Centralus resume required
• Centralus Epic Questionnaire required
