Job Overview

Required Qualifications
- Bachelor's degree in Physical Therapy
- Valid Physical Therapist license in Vermont
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- BLS
- Experience in a Home Health setting
Key Responsibilities
- Evaluate patients, develop treatment plans, and carry out therapy programs
- Screen, re-evaluate, and treat patients
- Communicate and coordinate care with the interdisciplinary team
- Document evaluations, treatment goals, and objectives
- Teach patients and family members about treatments and procedures
Work Environment
- Average patient visit is 45 minutes
- Broad independence with support from a cohesive care team
