Physical Therapist Home Health - Virginia (Travel/Contract)

• Location: Saluda, Virginia
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Shift: Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
• Hours per Week: 40 hours
• Average Patients per Day: 5-6
• Weekly Productivity Expectation: 30 points (points assigned based on visit type and activities)
• Travel Requirements: Must be willing to travel up to 75 miles from home clinic; approximately 200 miles traveled per week.
• Work Setting: Home Health (home visits with periodic office meetings)
• Types of Cases: Start of Care and Follow-Ups

Required Qualifications

• Valid Physical Therapist license in Virginia
• Minimum 1 year of experience as a Physical Therapist within the last year
• One completed travel assignment
• Experience working with adult patients
• Experience in a Home Health setting
• Current BLS certification

• On-Call and Weekend Requirements: Vary by location and should be discussed during the interview.
• Schedule Variations: Some positions may have a 4x10 schedule (Friday-Monday), but most are standard 5x8 Monday-Friday.
• Electronic Medical Record (EMR) System: Homecare Homebase (HCHB).
• Reason for Opening: Increased workload.

Compliance and Documentation Requirements

• Background Checks:

  • Standard 7-year background check including SS trace, county, nationwide, sex offender, and state-specific searches.
  • State police searches with notarized forms required for Virginia placements.
  • EPLS/GSA/SAM and HHS/OIG screenings required within 14 days of start and monthly thereafter.
  • Fingerprinting required per state regulations (varies by state).
• Education Verification: Degree verification required; transcripts or diplomas accepted if verification is unavailable.
• Health Requirements:

  • Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • Proof of immunity or vaccination for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, and COVID-19 (or documented declination).
  • 10-panel drug screen required.
  • Seasonal flu vaccination or documented declination required.
• References: Two professional references required for all placements.
• Work History Verification: Verification of at least two most recent employers or last 7 years if only one employer.

• Submit all required health documentation and immunization records.
• Complete any state-specific forms or releases required for placement.

This position is ideal for experienced Physical Therapists with recent adult home health experience who are comfortable with travel and working independently in patients' homes. Compliance with all licensing, certification, and background requirements is mandatory.
