Required Qualifications

• Valid RN license in Oregon
• Home Health (1 year of experience)
• Home Health experience within the last year
• One completed travel assignment
• BLS issued by American Heart Association or Canadian Heart & Stroke Foundation
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience in a Home Health setting
• Experience with OASIS

Key Responsibilities

• Home health visits in the West Salem/Keizer area and surrounding areas, with potential coverage for Woodburn
• May visit up to 6 patients per day
• Expected to have personal transportation
• Occasional travel radius may exceed 50 miles

Additional Notes

• First-time travelers are not accepted
• Local candidates are accepted within a 50-mile radius
• Primary series COVID vaccination plus one booster is required
• Epic experience is preferred
• Guaranteed hours may be reduced by up to 1 shift per pay period
