Medical Surgical LPN Travel/Contract Position

Location: Rochester, New York
Contract Length: 13 weeks
Shift: Night (7:00 PM – 7:00 AM), 3 shifts per week (36 hours/week)
Weekend Requirement: Every other weekend
Holiday Requirement: Available to work every other holiday, if needed

Job Summary

Provide nursing care to medical surgical patients including those with COPD exacerbation, infections, failure to thrive, and social issues. Must be able to take own patient assignment with RN support in an acute care setting. Floating within skillset required.

Required Qualifications

• Valid LPN / LVN license in New York (NY is not a compact state)
• Basic Life Support (BLS) certification issued by the American Heart Association
• Minimum of 1 year of Long Term Care experience within the last year
• Medical-Surgical nursing experience
• Experience with EPIC electronic health record system
• Ability to manage own patient assignment with RN support

Clinical Skills

• Vital signs, pain assessment, intake/output monitoring
• Isolation precautions, physical restraints, specimen labeling
• Oxygen administration via nasal cannula
• Feeding tube management
• Telemetry monitoring and 12-lead EKG
• Medication administration (oral, IM, SQ, NG/G-tube, ear and eye drops)
• IV therapy with pumps
• Blood glucose monitoring
• Basic wound care (simple dressings, JP/Hemovac drains)
• Bathing and mouth care
• Foley catheter insertion (male/female)
• Clean catch urine collection
• Communicating status changes to RN/MD
• Experience in medical-surgical and telemetry care settings

Unit Details

• Patient ratio (days): 1:5-6
• Patient population: Medical patients with COPD exacerbation, infections, failure to thrive, social issues
• Floating: Required within skillset
• Uniform: Navy blue and white scrub colors

Scheduling and Policies

• Shift hours: 7:00 PM to 7:00 AM
• Contract length: 13 weeks
• Weekend: Every other weekend required
• Holiday: Every other holiday availability required
• Call-off policy: Up to three shifts may be canceled by client without charge during contract
• Special scheduling requests must be submitted upon application
• First-time travelers accepted

Health and Compliance Requirements

• Government-issued photo ID required
• 7-year criminal background check within 3 months of start (including felony, SSN trace, sex offender, education verification)
• EPLS/GSA/SAM and HHS/OIG checks required within 3 months of start and run monthly
• Foreign education equivalency evaluation if applicable
• Education verification from primary source required
• Physical exam within 1 year of start, updated annually
• Fit mask test within 1 year of start, updated annually (specific mask types accepted)
• Negative TB test or chest X-ray within 1 year of start; TB questionnaire accepted in lieu of annual renewal
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug screen within 30 days of start
• Seasonal flu vaccination or signed declination required during flu season
• Snellen eye exam within 1 year of start
• COVID vaccination copy preferred but not required

Background and Verification

• Employment application required
• Work history verification required (minimum 2 past employments)
• References required for certain direct hire candidates (2 references within 3 years)
• Annual skills checklist required
• State-specific checks including sex offender registry and Medicaid exclusion list

Additional Notes

• Candidates must complete all health-related requirements and lab work within 5 days of contract signing
• Floating within skillset expected
• Locals accepted with adjusted pay rate
• Orientation and facility-specific compliance modules will be provided prior to start
