Job Opportunity: Case Manager (Registered Nurse)

Location

• Bel Air, Maryland

Employment Type

• Travel/Contract
• Contract Length: 13 weeks

Schedule

• Monday through Friday, Day Shift (5x8 hours)
• Shift hours: 8:00 or 8:30 AM to 4:00 or 4:30 PM (depending on needs)
• Holidays required as scheduled
• Floating required between two nearby facilities within a 10-mile radius
• Weekends not required but available if interested
• On-call not required

Job Description

• Inpatient case management focusing on coordinating medical home care, placements, social situations, and safe discharges
• Oversees guardianships, complex medical cases, and social wrap-around services
• Average daily caseload: approximately 15 patients
• EPIC charting experience required
• Business casual attire

Required Qualifications

• Valid Registered Nurse (RN) license in Maryland
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• Minimum 3 years of case management experience, including within the last year
• Experience in inpatient and acute care settings
• Experience with EPIC electronic health record system
