Physical Therapist - Home Health

Location: Warsaw, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide physical therapy services in a home health setting.
• Must be proficient in OASIS documentation.
• Business casual or solid color scrubs are permitted as dress code.
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM).

Required Qualifications

• Valid Physical Therapist license in Virginia.
• Minimum 1 year of Physical Therapist experience within the last year.
• Experience in a Home Health setting.
• Proficiency with OASIS documentation.

Compliance and Documentation Requirements

• E-Verify or attestation required.
• Photo for badge: recent, professional headshot meeting facility standards.
• Confidentiality agreement, dress code policy, safety brochure, drug-free policy, and hand washing hygiene policy must be reviewed and signed prior to start.
• Virginia State Police Check must be completed and cleared prior to start.
• TB screening: Negative TB test or chest X-ray within last 12 months; if positive, clearance letter signed by a physician required.
• Physical exam within the last 12 months required.
• Immunizations/vaccinations required or documented exemption for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (series, titer, or declination)
  • Seasonal Flu vaccination during flu season (10/1-3/31) with strict deadlines; medical or religious exemptions accepted with approval.
  • COVID vaccine series preferred; declination form accepted if no vaccine records.
• Pass/fail testing (e.g., medication/pharmacology competency) as applicable to profession, to be completed within one year of start date.
• Annual updates required for skills checklist and background checks.

Additional Notes

• Locals are allowed to apply at the same rate as travelers.
• Must be at least 21 years old to drive; 25 if transporting patients.
• Driving record must include at least 5 years of history from the state of license issuance.
• Fingerprinting requirements updated effective 7/15/2025; check with facility if needed.
• Orientation includes computer-based learning modules with multiple attempts allowed.
