Community Hospice Case Manager (Nursing) - Colorado, Grand Junction
Job Type
- Contract position
- Full-time, 40 hours per week
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM
- Occasional weekend backup and on-call duties
Job Description
- Provide compassionate hospice nursing care in patients' homes, nursing homes, group homes, or assisted living facilities
- Serve as a primary RN with an assigned caseload, leading the interdisciplinary care team
- Act as a teacher to patients and caregivers, helping identify personal goals of care and building caregiver confidence
- Manage symptom control and provide generalist care across all diseases, requiring strong assessment and critical thinking skills
- Work in an environment that supports personalized, life-changing hospice care
Required Qualifications
- Valid Registered Nurse (RN) license in Colorado
- Minimum 2 years of case management experience, including experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Hospice nursing experience
