Required Qualifications

• Valid Physical Therapist license in Indiana
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• BLS issued by American Heart Association

Preferred

• Tetanus/Diphtheria/Tdap

• Total staffed beds: 194, 210, 153, and 174 across listed areas

Scheduling Notes

• Facility is closed on Thanksgiving, Christmas, and New Year’s
• A crew will be scheduled the day after Thanksgiving, Christmas Eve, and New Year’s Eve
• Must work 2 of 3 major holidays
