Contract role in Missoula, Montana
Provides skilled nursing, care, and case management for patients in their residence or in an inpatient hospice setting

Admit patients to services and coordinate care to support continuity and quality outcomes
Assess, plan, implement, and evaluate nursing care using the nursing process
Oversee care provided by Home Health/Hospice Aides and CNAs
Maintain accurate patient records in accordance with agency standards and OASIS requirements
Participate in team scheduling, on-call rotation, quality improvement, and in-service training

Schedule varies
Typical in-home schedule is Monday through Friday, 8:00 AM to 5:00 PM, with on-call rotation
