Monday-Friday, 8:00 AM-5:00 PM, with on-call and weekend rotation

Role Summary

RN Case Manager provides skilled nursing and case management for patients in their place of residence. The role may start as a Visiting RN and move to Case Manager, with the same clinical responsibilities in both designations.

Required Qualifications

Valid RN license in Montana
Hospice experience (2 years)
Hospice experience within the last year
BLS
Associate of Science in Nursing / Associate Degree in Nursing
Driver's License
Home Health experience

Preferred Qualifications

BSN
2 years of recent acute care, home care, or hospice experience
Experience with venipuncture, catheterization, wound care, and/or parenteral therapy
HCHB experience

Key Responsibilities

Admit patients onto services
Coordinate and integrate services to support continuity of care
Assess, plan, implement, and evaluate nursing care
Provide patient and family instruction
Oversee care provided by Home Health Aides and CNAs
Maintain accurate patient records and OASIS requirements
Participate in team scheduling coordination and on-call coverage as needed
Participate in quality improvement activities and in-service training
