RN Care Manager

Overview

• Contract assignment in York, Maine
• 13 weeks
• 40 hours per week
• Variable shifts: 8:30 am to 4:30 pm or four 10-hour shifts starting at 7:30 am and ending at 5:00 pm
• No call
• Weekend and holiday rotation required

Role Summary

• RN Care Manager responsible for coordinating concurrent utilization management activities house-wide and serving as the main link between the patient, health care team, organization, and payors during the hospital stay

Required Qualifications

• Valid RN license in Maine
• Associate of Science in Nursing / Associate Degree in Nursing
• 3 years of Case Management experience
• Case Management experience within the last year
• Experience in a Home Care setting
• Care Manager experience
• Utilization Review experience
• Utilization Review (InterQual)
• Discharge Planning
• Active BLS

Preferred Qualifications

• Certification by a nationally recognized case management or utilization review organization
• CCM or ACM

Key Responsibilities

• Conduct patient assessments, identify resources needed for transition to the next setting, and secure those resources
• Collaborate with, educate, and lead an interdisciplinary team in daily care rounds, care conferences, and care planning
• Work with community organizations to secure durable medical equipment, health insurance authorizations, and transportation
• Participate in length-of-stay management for an inpatient hospital population
• Use strong written, verbal, and electronic communication skills
