Acute Rehabilitation Case Manager role in a 60-bed inpatient acute rehabilitation hospital.
Focus on treatment and recovery of patients with severe injury or illness.
Work Monday to Friday, 8:00 AM to 4:30 PM, no weekends or call required.
36-40 hours per week.

Acute freestanding rehabilitation hospital.
Patient ratio approximately 1:15.
Common diagnoses include stroke, post-surgical cases, and multiple medical debilitation conditions.
Average length of stay around 12 days.
Electronic Health Records used: UDS PRO, Medi Tech, Pinpoint.

Valid Registered Nurse (RN) license in Washington.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Minimum of 2 years of Acute Care Case Management experience, including experience within the last year.
Experience working with adult patients.
Utilization Review experience.
