Registered Nurse (Home Health) – Vermont

• Day shift: Monday to Friday with weekend rotation approximately every 4th week

Job Description

• Provide individualized, goal-oriented nursing care using the nursing process in accordance with physician orders.
• Serve as Case Manager when acting as the lead discipline in cases.
• Deliver home health and hospice care focused on holistic, community-centered support.

Required Qualifications

• Valid RN license for Vermont.
• BLS/BCLS certification.

Contract Length

• 12 weeks
