BLS certification issued by the American Heart Association
ARRT-CT certification

Current license/certification verification dated within 30 days of start
Fit test clearance
Completion of respirator evaluation with physician signature
COVID-19 vaccination proof including booster or approved declination/exemption (declination not accepted in CA or IL)
Proof of MMR vaccination or immunity
Hepatitis B vaccination series initiated prior to start (titers not accepted)
Influenza vaccine proof or declination (optional)
Tuberculosis screening: 2-step PPD or IGRA test; if positive, chest x-ray within last year for Connecticut
Varicella vaccination or immunity proof
Physical exam within the last year confirming fitness to work
10-panel drug screen completed within 30 days of start (no THC)
Signed code of conduct, confidentiality, conflict of interest, integrity and audit services, and patient information security agreements
Criminal background check covering federal, nationwide, and county records for the last 7 years, including SSN verification and alias checks, dated within 30 days of start
Education verification of highest academic degree or diploma
Exclusion screenings including EPLS, OIG, NSO, and OFAC within 30 days of start
Completion of Trinity skills checklist (provided by agency)
CORE exam within the last year
System access form completed and uploaded one week before start
Current Tdap vaccine or declination if no pediatric interaction
