Registered Nurse - Case Manager
Location
- Bozeman, Montana
Employment Type
- Contract
- Contract Length: 13 weeks
Schedule
- Day Shift: 7:30 AM – 4:30 PM
- Weekend rotations required
- May require working beyond a standard 40-hour workweek, including weekends and after-hours shifts
- On-call duties may be required
Job Summary
- Collaborate with physicians and multidisciplinary teams to develop and monitor patient care plans
- Conduct comprehensive assessments to identify patients who qualify for case management
- Develop and implement case management plans addressing patient needs and goals
- Monitor care plans for effectiveness, document interventions, and suggest changes
- Manage ongoing caseload with regular outreach
- Promote integration of behavioral health and long-term services to enhance continuity of care
- Actively participate in discharge planning
Required Qualifications
- Montana Registered Nurse (RN) licensure
- American Heart Association Basic Life Support (BLS) certification
- Minimum 2 years of nursing experience
- Utilization review experience is mandatory
Preferred Qualifications
- Bachelor’s Degree in Nursing
- 2 years of acute care hospital experience as a case manager

- Sound judgment, patience, and professional demeanor
- Ability to work in a busy, stressful environment
- Strong interpersonal, verbal, and written communication skills
- Tact, discretion, sensitivity, and confidentiality
- Proficiency with computer applications including MS Office and electronic medical records (EMR)
- Ability to analyze, organize, and prioritize work to meet multiple deadlines
- Strong emotional intelligence and teamwork skills
Physical Requirements
- Frequent lifting up to 50 pounds
- Ability to sit, stand, and walk for extended periods
- Occasional climbing of stairs or ladders
- Frequent twisting, bending, stooping, kneeling, reaching above shoulder level, pushing, and pulling
- Continuous fine finger movements and close visual acuity
- Cognitive ability to learn new tasks, maintain focus, and make timely decisions
- Exposure to bloodborne pathogens, radiation, chemicals, infectious diseases, and emotionally challenging situations
Additional Information
- Candidates must live at least 90 miles away from the facility to be considered
- Facility reserves the right to cancel up to three shifts during the 13-week contract due to low census or unforeseen circumstances
