Physical Therapist - Home Health (Travel/Contract)
Location: Fredericksburg, Virginia (with occasional travel 1-2 days/week to Falls Church area)
Contract Length: 13 weeks
Shift: Day shift, 8:00 AM – 5:00 PM (5 days/week, 8 hours/day)
Hours per Week: 40 hours guaranteed
Job Overview
- Caseload primarily Home Health patients aged 18+
- Average 4-6 patients per day
- Minimum 24-30 visits per week required to qualify for 40-hour guarantee
- Mostly home visits with 1-2 patients seen weekly in clinic
- Charting must be completed onsite using Kinnser/Wellsky; no remote charting allowed
- Free residential parking available
- Business casual dress code with closed-toe shoes required
- Mileage reimbursed at IRS standard rate (from first to last patient, not from/to home)
- Locals within 50-mile radius accepted
Required Qualifications
- Valid Physical Therapist license in Virginia
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification required at time of submission
Work Schedule & Requirements
- No weekend work required; weekends optional with advance notice
- No on-call duties
- Must be punctual, self-motivated, and able to complete charting efficiently
- Strong communication skills and patient-focused attitude highly valued
Compliance & Documentation (Must be completed prior to start)
- Government-issued photo ID (Driver’s License/State ID)
- Emergency Contact Form
- Background checks including SS trace, county searches (last 7 years), nationwide, sex offender registry (within 1 year)
- Annual EPLS/GSA/SAM and HHS/OIG checks
- Certifications: BLS (required), ACLS and PALS preferred but not mandatory
- State license primary source verification
- Competency exams as applicable
- Physical exam within 1 year
- Negative TB test or chest X-ray within 1 year (with special rules if past positive)
- Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella as per policy
- 9-panel drug screen within 1 year (instant drug screen acceptable)
- Flu vaccination or documented declination required during flu season (Oct 1 - Mar 31)
- Two professional references required (travel, allied, or per diem acute care) within 3 years
Additional Notes
- Compliance processing may take 2-3 weeks; all required documents must be submitted before starting patient care
- All required Return to Office (RTO) documentation must be submitted with application; no additions accepted after offer
- Covid vaccine declination allowed
