Job Overview

Required Qualifications

• Valid CT Technologist license in Arizona
• CT Technologist experience within the last year
• One completed travel assignment
• BLS - Basic Life Support issued by American Heart Association or American Red Cross
• ARRT
• Experience with Meditech

Documentation and Compliance

• Government-issued photo ID
• Background check with 7-year search within 30 days of start; must be updated annually
• EPLS/GSA/SAM check, annually
• HHS/OIG check, annually
• Transcript or diploma
• Education verification through NSCH
• Facility-specific job description
• Facility-specific forms and training, including confidentiality, behavioral expectations, right to know, HIPAA/information security, and agency candidate form
• Photo for badge
• Physical exam within 1 year of start
• Fit test within 1 year of start; updated annually
• Negative TB test or chest X-ray within 1 year of start; annual TB questionnaire required
• Measles, mumps, and rubella immunity documentation
• Hepatitis B documentation
• Varicella documentation
• 9-panel drug screen within 30 days of start
• Flu vaccination or declination during flu season
• 2 references within 3 years of start
• Skills checklist annually

Additional Notes

• Meditech charting required
• ACLS, PALS, and NRP are listed as applicable certifications
• Competency exam and medication/pharmacology competency exam are listed as applicable requirements
