Hospice Nurse

Job Overview

• Location: Sunnyvale, California
• Employment type: Contract
• Contract length: 13 weeks
• Schedule: Days, Sunday–Thursday; 10am–7pm or 11am–8pm
• Hours per week: 40
• Service area: San Francisco County, San Mateo County, and Santa Clara County

Role Summary

• Admits hospice patients and assesses physical, psychosocial, environmental, safety, and developmental needs
• Develops individualized plans of care and documents observations and interventions
• Coordinates care with a multidisciplinary team
• Participates in quality assessment and performance improvement activities
• Conducts follow-up visits as needed

Required Qualifications

• Valid RN license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Hospice experience
• Case Management (1 year of experience)
• Case Management experience within the last year
• BLS
