Home Health Registered Nurse (RN) - Travel/Contract

Location: Concord, California (Territory includes Walnut Creek, Lafayette, Orinda, Moraga, San Ramon - up to 60-mile radius)
Contract Length: 13 weeks

Job Description

• Provide home health nursing care primarily to a geriatric patient population.
• Manage approximately 30 patient points per week (about 5-6 patient points per day).
• Work schedule: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM).
• Guaranteed 40 hours per week (productivity requirements apply).
• Two weekend days required per month.
• No on-call duties.
• Flexibility with assigned territory is expected.
• Scrubs required (any color), no jeans.
• iPad provided; cell phone not provided.
• Mileage reimbursed at IRS standard rate.
• First-time travel nurses accepted; local candidates accepted at travel rate.

Required Qualifications

• Valid Registered Nurse (RN) license in California.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Minimum 1 year of recent Home Health nursing experience within the last year.
• Experience in a Home Health setting.
• Experience with OASIS (Outcome and Assessment Information Set).
• Experience with Homecare Homebase (HCHB).
• BLS (Basic Life Support) certification issued by American Heart Association or American Red Cross.
• ACLS (Advanced Cardiac Life Support) certification issued by American Heart Association or American Red Cross.

Health and Compliance Requirements

• Physical exam within 6 months prior to start.
• Negative TB test (PPD, Quantiferon Gold, or T-spot) within 6 months of start; chest X-ray acceptable within last 5 years if history of positive TB.
• Proof of immunity or vaccination for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (vaccine series or declination required)
  • Varicella (titer, vaccination, or history of chickenpox)
• Seasonal flu vaccination or declination required during flu season (October 1 - March 31).
• 9 or 10 panel drug screen within 30 days to 1 year of start (instant drug screens acceptable).

Background and Documentation

• Government-issued photo ID (Driver’s License or State ID).
• Emergency contact form required prior to start.
• Background check within 30 days including:

  • Federal and county criminal searches (last 7 years)
  • Sex offender registry
  • FACIS Level 3
  • DMV Motor Vehicle record
• Annual EPLS/GSA/SAM and HHS/OIG checks required.
• Education verification (diplomas/transcripts or third-party verification).
• Verification of last 3 employers required.
• Two professional references required (within 3 years of start).
• Annual skills checklist required.

Additional Notes

• No prior travel experience required.
• Productivity standards must be met to maintain guaranteed hours.
