Home Health Registered Nurse (RN) - Travel/Contract
Location: Concord, California (Territory includes Walnut Creek, Lafayette, Orinda, Moraga, San Ramon - up to 60-mile radius)
Contract Length: 13 weeks
Job Description
- Provide home health nursing care primarily to a geriatric patient population.
- Manage approximately 30 patient points per week (about 5-6 patient points per day).
- Work schedule: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM).
- Guaranteed 40 hours per week (productivity requirements apply).
- Two weekend days required per month.
- No on-call duties.
- Flexibility with assigned territory is expected.
- Scrubs required (any color), no jeans.
- iPad provided; cell phone not provided.
- Mileage reimbursed at IRS standard rate.
- First-time travel nurses accepted; local candidates accepted at travel rate.
Required Qualifications
- Valid Registered Nurse (RN) license in California.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum 1 year of recent Home Health nursing experience within the last year.
- Experience in a Home Health setting.
- Experience with OASIS (Outcome and Assessment Information Set).
- Experience with Homecare Homebase (HCHB).
- BLS (Basic Life Support) certification issued by American Heart Association or American Red Cross.
- ACLS (Advanced Cardiac Life Support) certification issued by American Heart Association or American Red Cross.
Health and Compliance Requirements
- Physical exam within 6 months prior to start.
- Negative TB test (PPD, Quantiferon Gold, or T-spot) within 6 months of start; chest X-ray acceptable within last 5 years if history of positive TB.
- Proof of immunity or vaccination for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (vaccine series or declination required)
- Varicella (titer, vaccination, or history of chickenpox)
- Seasonal flu vaccination or declination required during flu season (October 1 - March 31).
- 9 or 10 panel drug screen within 30 days to 1 year of start (instant drug screens acceptable).
Background and Documentation
- Government-issued photo ID (Driver’s License or State ID).
- Emergency contact form required prior to start.
- Background check within 30 days including:
- Federal and county criminal searches (last 7 years)
- Sex offender registry
- FACIS Level 3
- DMV Motor Vehicle record
- Annual EPLS/GSA/SAM and HHS/OIG checks required.
- Education verification (diplomas/transcripts or third-party verification).
- Verification of last 3 employers required.
- Two professional references required (within 3 years of start).
- Annual skills checklist required.
Additional Notes
- No prior travel experience required.
- Productivity standards must be met to maintain guaranteed hours.
