Completion of orientation modules (approx. 4 hours) prior to start; failure to complete results in being sent home
Submission of a clear, passport-style photo for badge
Signed agreements prior to start:
- Assignment acknowledgment
- Code of conduct
- Employee confidentiality agreement
- Systems user agreement
- HIPAA confidentiality agreement
User access request and employee information forms completed before start
Facility-specific COVID exemption form required if requesting religious exemption
Radiation badge process must be initiated before start; badge delivery may take 5-7 days
