Medical Surgical LPN Travel/Contract Job - Rochester, NY

Position Details

• Job Title: Medical Surgical LPN
• Location: Rochester, New York
• Employment Type: Travel/Contract
• Shift: Night shift, 7:00 PM – 7:00 AM (3 shifts per week, 12 hours each)
• Contract Length: 13 weeks
• Weekend Requirement: Every other weekend
• Holiday Requirement: Must be available to work every other holiday, if needed
• Charting System: EPIC experience required
• Floating: Required to float within skillset
• Patient Ratio: Approximately 1:5–6
• Unit Description: Complex care unit for medicine, orthopedic overflow, and complex long-term patients
• Scrub Color: Navy blue and white combinations

Required Qualifications

Licenses & Certifications

• Valid LPN / LVN license in New York (NY is not a compact state; pending licenses not accepted)
• Basic Life Support (BLS) certification issued by American Heart Association

Experience

• Long Term Care experience within the last year (minimum 1 year)
• Medical Surgical experience
• Must be able to take own patient assignment with RN support

Clinical Skills

• Blood product administration
• PICC line management
• High Flow Nasal Cannula (HFNC)
• Tracheostomy care/management
• Heparin protocols
• Use of rapid response/code teams
• Medical-Surgical/Telemetry monitoring
• Orthopedic trauma care

Technical Skills

• Experience with EPIC charting system

Requirements & Compliance

Background Checks & Verifications

• 7-year criminal background check (felony checks) within 3 months of start
• Social Security Number trace, national criminal, sex offender registry, education verification from primary source
• EPLS/GSA/SAM and HHS/OIG checks within 3 months of start and monthly thereafter
• Foreign equivalency education evaluation if applicable
• Employment application and work history verification (minimum 2 past employments)
• References (2 within 3 years) required for travel agency compliance

Health & Safety Requirements

• Physical exam within 1 year of start, updated annually
• Fit mask test within 1 year of start, updated annually (specific acceptable mask models)
• Negative TB test or chest X-ray within 1 year of start; TB questionnaire accepted in lieu of annual renewal
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (surface antibody positive titer, completed series, or declination)
  • Varicella
• Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
• Seasonal flu vaccination or signed declination required during flu season
• Snellen eye exam within 1 year of start
• COVID vaccination copy preferred but not required

Facility-Specific Documents

• Government-issued photo ID required
• Passport-style photo for badge
• Completion of orientation safety manual acknowledgment
• Primary source verification of state license within 30 days of start
• NURSYS verification within 30 days of start

State-Specific Requirements (New York)

• Sex offender registry search within 30 days of start
• Medicaid exclusion list check within 3 months of start and monthly thereafter
• Fingerprinting and additional background checks may be required for certain units (mental health, behavioral health, home health aide, long-term care)

Additional Notes

• First-time travelers are accepted
• Local candidates accepted with adjusted pay rate
• The client may cancel up to three shifts during the contract without penalty
• Requests for time off, block scheduling, and special scheduling must be submitted at time of application

This position offers an opportunity to work in a complex care environment with a variety of acute care nursing responsibilities on a night shift schedule. Candidates must meet strict licensure, certification, and health compliance requirements.
