Transform Palliative Services.  We are seeking a compassionate and experienced Advanced Illness Management (AIM) Palliative Registered Nurse to join our team. Reporting to the Director of Palliative Care, this role provides high-quality palliative care to patients in their homes, assisted living facilities, and skilled nursing facilities. The AIM Palliative RN collaborates with patients, families, and interdisciplinary team members to assess, educate, and coordinate care while ensuring comfort and quality of life. Essential Functions of Position:<ul><li>Conduct in-person and telephonic RN assessments and screenings for palliative care patients.</li><li>Complete Risk Stratification Assessments to identify patient support needs in consultation with the Nurse Practitioner.</li><li>Observe patient visit frequency based on individual needs and risk identification.</li><li>Educate patients and families on disease processes, self-care, end-of-life care, medications, nutrition, and treatments.</li><li>Communicate findings to the Palliative Nurse Practitioner and/or PCP, including changes in condition or signs of decline.</li><li>Coordinate care with palliative team members and support hospice referral as appropriate.</li><li>Document timely clinical records including assessments, progress notes, care plans, physician orders, and treatments.</li><li>Support care plan development from initial assessment through discharge, addressing functional decline, symptom management, nutrition, cognition, and family/caregiver needs.</li><li>Collaborate on modifications to individualized palliative plans of care.</li><li>Facilitate care delivery and obtain additional resources as needed in accordance with the Plan of Care.</li><li>Participate in evening/weekend on-call rotation as required.</li><li>Facilitate prescription transmission to pharmacies, manage lab and diagnostic results, and request durable medical equipment as medically necessary.</li><li>Maintain effective relationships with supportive care team members and other healthcare providers.</li><li>Communicate and document clinical findings, treatment plans, and care provided in accordance with nursing standards and regulatory requirements.</li><li>Adhere to company policies, HIPAA privacy standards, business ethics, and mandatory compliance programs.</li><li>Implement marketing and promotional initiatives as directed.</li><li>Perform other related duties as assigned.</li></ul>

We support patients and their families with dignity, comfort, and love. Full-time Weekday position with limited after hours required.  We are open to 5 8's or 4 10 hour days.   We offer a supportive team, mileage reimbursement/fleet vehicle, comprehensive benefit package, competitive annual salary and great work life balance.  What You'll Do as a Hospice RN / RN Case Manager:<ul><li>Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.</li><li>Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care—in their home, assisted living, or wherever they call home.</li><li>Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.</li><li>Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to build and refine personalized care plans.</li><li>Provide emotional, physical, and spiritual support not just to the patient, but to their loved ones as well—offering education, comfort, and bereavement guidance.</li><li>Teach caregivers how to care for their loved one safely and confidently, offering both practical training and emotional reassurance.</li><li>Communicate important updates in patient condition to the care team, ensuring timely and appropriate interventions.</li><li>Maintain accurate, up-to-date clinical documentation and follow infection control and compliance standards.</li><li>Support the organization's quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient.</li><li>Help patients and families understand and navigate topics such as:   • Medication administration   • Hospice philosophy and services   • Symptom and pain management   • End-of-life processes and expectations</li></ul>

About You:
Qualifications – What You’ll Bring:<ul><li>Active RN license in the state of employment (or eligible to obtain).</li><li>1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.</li><li>A genuine hospice heart: compassionate, empathetic, and patient-centered.</li><li>Comfortable providing care in diverse settings, including private homes and facilities.</li><li>Strong communication and critical thinking skills in emotionally sensitive situations.</li><li>Valid driver’s license, auto insurance, and reliable transportation.</li><li>CPR certification required.</li></ul>Preferred Experience (Not Required):<ul><li>Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)</li><li>Admissions RN, wound care, float pool, travel nursing</li><li>Experience with terminally ill patients or serious illness support</li></ul>
