CNA Reciprocity and Certification Requirements by State

Steps to Becoming a CNA

Some states call CNAs something else, such as licensed nursing assistants, and a few have two levels of certified nursing assistants. The steps to becoming a CNA are similar in most states, but some may have unique regulations and requirements. 

Prospective CNAs must usually complete a state-approved training program at an accredited institution. Training programs may last between 1 and 4 months, but accelerated programs allow you to finish in 4 weeks. Programs typically include classroom and clinical training hours. 

Each state usually provides a list of approved CNA training programs but often accepts training programs completed in other states if they meet their standard requirements. CNA programs typically require prospective students to possess a high school diploma or GED equivalent and current immunizations, including the Hepatitis B series and a TB skin test. Some programs may require students to pass a criminal background check and drug screening.

Once you complete a training program, you must pass a state competency exam. These tests involve a written or oral exam and a skills evaluation exam, and applicants must pass both before the state adds them to its CNA Registry. You must pass your CNA certification exams within a set period following CNA program completion, usually within 24 months. Some states allow you to challenge the certification exam without completing a state-approved training program if you meet specific qualifications. 

The National Nurse Aide Assessment Program (NNAAP), developed by the National Council of State Boards of Nursing (NCSBN,) is the largest nurse aide certification examination program in the United States. As of early March 2024, 18 NCSBN jurisdictions used the NNAAP exam for testing CNAs, including 13 states (California counts as two jurisdictions), three U.S. territories and the District of Columbia.

Nearly all states require some version of a CNA certification application, but not all charge an application fee. Many require fingerprinting for a background check, which usually requires a fee, even if the application process doesn’t require one. 

You might pay a testing fee, but federal and/or state laws may require certain employers to cover this cost. If you’re already employed or have been offered a travel, staff, per diem or local contract CNA job, confirm whether the employer is responsible for testing and retesting charges. 

Certification typically lasts for 2 years. Before expiration, you must follow the state’s requirements for renewal. If you’ve earned reciprocal certification in another state, you must also renew this to continue traveling to and working in any state you have reciprocity.

When a state offers reciprocity, it agrees to allow a CNA certified in good standing in another state to practice without further training or testing requirements. Alabama

CNA Certification Requirements

  • Complete a CNA program approved by the Alabama Department of Health that has at least 75 clock hours of training, including 16 hours of clinical training and 16 hours of labs 
    • May also qualify by completing equivalent training through higher education, such as an LPN or RN program approved by the Alabama Board of Nursing or a state-approved training program at a long-term care facility
    • Online CNA programs not approved in Alabama
    • Must be CPR-certified before starting clinical training (Basic Life Support certification recommended)
  • Pass the Nurse Aide Competency Exam administered by Prometric or the NNAAP exam administered by Credentia
    • Failure to pass the competency exam within three attempts and 24 months requires retaking a State-approved nurse aide training program before retesting

Application Fees

Alabama doesn’t have an application fee. CNA training is free through an approved nursing home, but costs vary if completing training through a college, trade school or other approved training program.

Exam Fees:

  • Prometric: $40 Written/Oral Exam and $45 Clinical Skills Exam
  • NNAAP: $55 Written/Oral Exam and $80 Skills Evaluation

Renewal Requirements: No Fee

Alabama doesn’t require a renewal application, continuing education (CE) or other renewal requirements. However, to maintain CNA certification, you must:

  • Work at least 8 hours every 24 months in a paid role at a nursing home, hospital, home health agency, hospice agency or mental health facility

Although CE isn’t required to maintain certification, skilled nursing facilities must provide 12 mandatory hours of CE for CNAs each year.

CNA Reciprocity: No Fee

Alabama doesn’t charge a fee or have a form for CNAs to fill out for reciprocity. Instead, CNAs certified in other states must have their employers contact the State agency and have their name placed on the Alabama Nurse Aide Registry. Thus, the only way to get on the state’s registry is to apply for a job at a nursing facility and have it submit your name once it verifies your active certification in the state you’re transferring from. You can’t transfer your CNA certification yourself.

Alaska

CNA Certification Requirements

  • Complete approved CNA program with a minimum of 140 training hours, with at least 60 classroom hours and 80 hours of lab/clinical practice
  • Complete approved CNA program with a minimum of 140 training hours, with at least 60 classroom hours and 80 hours of lab/clinical practice
    • May also qualify through equivalent nursing education of at least one year that included classroom and clinical training and occurred within the last 2 years, military training and service as a U.S. corpsman or medic within the past 5 years, a nursing training program in a foreign country or possession of a current or lapsed unencumbered registered nursing license
  • Complete fingerprinting for criminal background checks through the Department of Public Safety and the FBI
  • Submit Alaska’s Nurse Aide Certification by Examination application
  • Pass the NNAAP exam administered by Credentia

Applicants behind on child support payments per the Alaska Child Support Enforcement Division receive a nonrenewable 150-day temporary CNA license to use while resolving payment issues.

Renewal Requirements: $100 Fee

  • Certificates are valid for 2 years and expire on March 31 of every even-numbered year.
  • Must complete 24 contact hours of continuing education (CE) and 160 hours of paid employment as a CNA or similar role.

CNA Reciprocity: $100 Application, $100 Certificate, $75 Fingerprinting

Applicants may qualify for a nurse aide by endorsement in Alaska if they have proof they completed a state-approved CNA training program that meets minimum federal standards, possess a current, unencumbered nurse aide certification or registration issued by any state, U.S. territory or Canadian province or territory and passed a competency evaluation.

Arizona

CNA Certification Requirements

The Arizona State Board of Nursing has two levels of nursing assistants:

Certified Nursing Assistant:

  • Complete a board-approved CNA program with at least 120 CNA training hours
  • Pass the state’s written and manual skills exam
  • Submit proof of legal residency

Licensed Nursing Assistant:

  • Meet CNA education and exam requirements
  • Submit fingerprints for background check
  • Submit proof of legal residency
  • License renews every 2 years

  • CNA: No Fee
  • LNA: $50 license fee and $50 fingerprint fee

Renewal Requirements: No Fee CNA / $50 Fee LNA

CNA:

  • Must work a minimum of 8 hours in the past 2 years

LNA:

  • License requires renewal every 2 years
  • Must work at least 160 hours in the past 2 years

CNA Reciprocity: $50 CNA / $100 LNA

Nursing assistants from other states may apply for endorsement as a CNA for a $50 fee or an LNA for a $100 fee ($50 license and $50 fingerprinting). All applicants must:

  • Complete the CNA Endorsement application
  • Provide proof of graduation from a comparable nursing assistant program
  • Pass the state nursing assistant written and manual skills competency exams
  • Be listed on another state’s CNA registry
  • Worked with the past 2 years

LNAs must also submit fingerprints for a background check.

Arkansas

CNA Certification Requirements

  • Complete a state-approved CNA program with at least 90 hours of education/training
  • Create an Arkansas TMU account
  • Pass the Nursing Assistant Competency Exam within one year of completing training and within 3 attempts

CNAs may be placed on the Arkansas Nurse Aide Registry (ARNAR) through the out-of-state registry process if they’re in current, good standing on the NAR in another state.

Applicants who’ve completed a nurse aide competency exam that’s the same or substantially similar to Arkansas’ exam within a year of completing a training program of at least 90 hours

Applicants who haven’t passed a competency exam must pass the Arkansas competency exam within one year of completing their training

CNA Certification Requirements

The Healthcare Workforce Branch, Healthcare Professional Certification and Training Section is primarily responsible for certifying nurse assistants.

  • Be at least 16 years old
  • Complete a CNA training program approved by the California Department of Health that includes knowledge and skills test
  • Submit electronic fingerprints through Live Scan
  • Obtain a criminal record clearance
  • Complete the Certified Nursing Assistant Initial Application

CNA Certification Requirements

CNA Certification Requirements

  • Complete a Delaware Division of Long Term Care Residents Protection-approved CNA training program

CNA Certification Requirements

  • Submitting proof of CNA certification, employment and government-issues social security card and photo ID
  • Submitting proof of paid employment as a CNA for at least 8 hours under RN supervision within the past 2 years

The Georgia Nurse Aide Registry verifies registry status on the Nurse Aide Registry in the state CNAs list on their applications, then evaluates completed application and state verification information before adding the CNA’s name to the Georgia registry without taking the Georgia Written/Oral and Skills Competency Examination.

CNA Certification Requirements

The Hawaii Department of Commerce and Consumer Affairs contracts with Prometric to develop and administer its Nurse Aide Competency Exam and manage the Nurse Aide Registry.

  • Complete an approved CNA training program with 100 education hours
  • Submit a copy of training completion document from a Hawaii-approved 100-hour training program with application
  • Complete the Hawaii Nurse Aide Examination Application form and send it to Prometric
  • Pass both parts of the Hawaii Nurse Aide Competency Exam administered by Prometric within 2 years of the first testing date or within 3 attempts or retraining required

Once you pass the exam, the CNA certificate will be mailed to you and your name will be added to the Hawaii Nurse Aide Registry.

CNA Certification Requirements

The Idaho Department of Health and Welfare utilizes Prometric to administer its written Certified Nurse Aide exam.

  • Complete a state-approved CNA training program that includes a minimum of 120 training hours
  • Pass the Manual Skills evaluation within 6 months of completing a training program
  • Bring proof of completion when taking the Idaho Nurse Aide exam within 12 months of completing a training program
    • Written exam: $29 plus testing site fees
    • Oral exam: $39 plus testing site fees

Renewal Requirements: No Fee

  • Must work at least 8 hours as a paid CNA in the 2 years before expiration date
    • Volunteer hours don’t count

Reciprocity: No Fee

Reciprocity is only granted to CNAs offered employment or living in Idaho.

CNA Certification Requirements

  • Complete an Illinois-approved CNA training course
    • Must include 80 hours of education and 40 hours of clinicals, with at least 12 hours focused on dementia
  • Complete a criminal history background check
  • Pass the written Nursing Assistant Proficiency Exam administered by the Southern Illinois University at Carbondale within 3 attempts

Renewal Requirements: No Fee

CNA certification doesn’t expire, but CNAs must continue to provide nursing or nursing-related services for pay. If CNAs don’t work in this capacity for more than 24 months, their certification becomes inactive and they must retake the manual skills and competency tests to become recertified.

Reciprocity: $25 Fee

Applicants deemed competent on another state’s CNA registry may apply for reciprocity if they:

  • Complete the Out-of-State Nurse Aide Application to Become an Illinois Certified Nurse Aide
  • Provide documentation of current registration showing Code of Federal Regulations requirements have been met
  • Don’t have any administrative findings of neglect, abuse or misappropriation of property on their home state’s or any other state’s CNA registry
  • Don’t have any disqualifying convictions on their home state’s CNA registry

CNA Certification Requirements

Iowa

CNA Certification Requirements

Iowa places CNAs, also called nurse’s aides, on its Direct Care Worker Registry. CNAs must apply online to be added to this registry to be eligible to work in long-term care facilities in the state. However, CNAs are only required to be on the registry if they’re employed in nursing homes, skilled nursing facilities or skilled or swing-bed units of hospitals. 

To apply, CNAs must:

  • Complete a 75-hour state-approved CNA training program
  • Pass the written and skills components of the CNA exam within 3 attempts and 4 months of hire date
    • $60 written exam
    • $115 skills exam

Once Registry staff receive notification that an applicant passed the exam from the testing entity, they place the CNA on the registry. CNAs may print their Registry cards from the Iowa Direct Care Worker database.

Renewal Requirements: No Fee

  • Must perform at least 1 day of nursing or nursing-related duties every 24 months
  • Must complete at least 12 hours of in-service training annually at long-term facilities

Reciprocity: No Fee

CNA Certification Requirements

CNA Certification Requirements

CNAs are called State Registered Nurse Aides (SRNAs) in Kentucky. The Kentucky Community and Technical College System (KCTCS) is the state’s sole provider of nurse aide testing, with testing available at various colleges throughout the state. 

  • Complete an approved CNA course consisting of a minimum of 75 hours, with 16 hours of supervised clinical training
  • Create a Kentucky Board of Nursing Nurse Portal account

Louisiana

CNA Certification Requirements

The Louisiana Department of Health (LDH) – Health Standards Section manages the Louisiana Nurse Aide Registry and the Direct Service Worker Registry. The LDH also administers the state’s nurse aide program and contracts with Prometric to develop and administer its CNA Competency Evaluation Program. Applicants must:

  • Complete a state-approved CNA training program, including 40 hours of classroom education and 40 hours of clinical training within the last 12 months
  • Complete the Louisiana Nurse Aide Application form on Prometric
  • Pass the Clinical Skills exam and Written or Oral exam to earn CNA certification
    • $125 written/oral and clinical skills exams
    • $40 written/oral exam only retest
    • $85 clinical skills exam only retest

Prometric sends successful candidates’ information to LDH to be placed on the Louisiana Registry. Candidates also receive a Certification of Completion from Prometric via email to the address provided on their applications. 

Renewal Requirements: No Fee

CNAs are automatically recertified if the required work history is reported to the Nurse Aide Registry before the certification period ends. Nurse aides without the required work history may be eligible to test for recertification.

Reciprocity: $35 Fee

Prometric also handles the processing of reciprocity applications for Louisiana. Applicants must:

  • Be active and in good standing in all states in which they’re currently certified
  • Complete the application found under the Candidate Resources section of the Prometric website using the Route 8 Reciprocity option 
  • Submit a copy of social security card and government-issued Louisiana ID or military ID 

Applicants with addresses within 30 miles of Louisiana won’t be required to obtain a Louisiana ID to process their reciprocity application. Once Prometric verifies an application, it sends the information to LDH, which places the applicant’s name on the Louisiana Nurse Aide Registry.

Maine

CNA Certification Requirements

  • Be at least 16 years old
  • Complete 130 hours through a Maine-approved CNA training program
    • 70 hours classroom theory
    • 20 hours clinical lab work
    • 40 hours supervised clinical practice
  • Pass the Maine CNA competency test consisting of a written or oral exam and a skills demonstration
    • Written/oral exam may be administered directly by the State or a State-approved entity that’s not a nursing or skilled nursing facility that participates in Medicaid
    • Skills demonstration must be performed in a facility comparable to the setting where the applicant will function as a nurse aide
    • Competency tests are given once a month in Auburn, Bangor, Biddeford, Caribou and Houlton with a $200 fee
  • Pass a criminal background check
  • Complete and submit the CNA Application with appropriate documentation attached

Applicants may also complete “Non-Traditional CNA Competency Testing Evaluation” testing.

Maryland

CNA Certification Requirements

  • Complete 100 hours of CNA instruction through a Board-approved CNA program
  • Pass the CNA competency examination (written and manual skills) offered through Credentia and the Maryland Geriatric Nursing Assistant Testing Services (MDGNATS)

CNA certification is the basic level of certification in Maryland, with other certifications in addition to this certification. Applicants must complete the CNA certification first or in conjunction with additional certifications, including:

  • Geriatric Nursing Assistant (GNA) for all nursing assistants working in licensed comprehensive care facilities
  • Home Health Aide (HHA) required to work in licensed home health agencies
  • Certified Medicine Aide (CMA) required to administer medications in a licensed comprehensive care facility
    • Only GNAs with 1 year of experience and completion of an approved 60-hour medicine aide course taught in a community college can become a CMA

All certifications appear on the CNA certification card; most reciprocating states require GNA certification. The Susquehanna Red Cross (MDGNATS) offers exams and tells the Board of Nursing who passed. There’s no cost for GNA certification or renewal if the applicant meets GNA certification requirements. GNA certification expires at the same time as CNA certification.

Applicants can attempt the written or audio knowledge test up to 4 times and the skills test up to 3 times. Those who don’t pass either test in the maximum number of attempts must complete a new Massachusetts Department of Public Health-approved training program to become eligible to attempt the Massachusetts NA exams again.

Allow at least 3 weeks to receive test results and for your name to appear on the license verification website.

Michigan

CNA Certification Requirements

Effective July 26, 2023, the Michigan Department of Licensing & Regulatory Affairs transitioned its Nurse Aide Registry services to the Bureau of Community and Health Systems (BCHS), calling the new system Michigan Nurse Aide, Training, and Enforcement System (MI-NATES). D&S Diversified Technologies/Headmaster provides nurse aide testing services. 

Applicants must:

  • Complete an approved Michigan nurse aide course that includes 75 hours of training
  • Pass the written/oral and clinical evaluation exam administered through Headmaster  within 24 months of course completion
  • Create a MILogin account
  • Request access to MI-NATES and set up a user profile

Application Fees

  • $40 registration fee
  • $175 written knowledge/audio and skills exams fee (each attempt)

Renewal Requirements: $40 Fee

  • Log into your account in MI-NATES and submit a renewal application
  • Acknowledge proof of paid CNA employment providing nursing or nursing-related services for at least 40 hours in the last 24 months
  • Submit attestation of completion of at least 24 hours of continuing education annually (Effective March 23, 2026)

Reciprocity: $20 Fee

Nurse aide candidates from approved states may request reciprocity if their training program is consistent with the Federal Code of Regulations and includes 75 course hours before taking a competency evaluation exam. Michigan reciprocity doesn’t allow candidates to exempt out of training course hours or testing. Fill out the Out-of-State Training Waiver for Michigan Test Eligibility and pass them exam once deemed eligible to test.

The State of Michigan grants reciprocity from CNAs certified in 37 states. It doesn’t grant reciprocity to those certified in Delaware, Florida, Maryland, Massachusetts, Michigan Minnesota, Montana, North Carolina, Rhode Island, Vermont, Virginia, West Virginia and Wyoming.

Minnesota

CNA Certification Requirements

Minnesota automatically adds nursing assistants who complete an approved nursing assistant training and competency evaluation program in Minnesota or pass the Minnesota-approved competency test for nursing assistants to its Nursing Assistant Registry (NAR).

Minnesota contracts with D&S Diversified Technologies/Headmaster to place candidates on the NAR. Minnesota State Colleges and Universities administers the Nurse Aide Skills and Knowledge tests for a $275 fee for both. Applicants can take the knowledge test up to 4 times and the skills test up to 3 times.

Renewal Requirements: No Fee

  • Work at least 8 hours as a paid nursing assistant every 24 months
  • Complete the Nursing Assistant Registry Update form
  • Provide a copy of a recent pay stub from employer

Reciprocity: No Fee

CNAs in good standing on the Nursing Assistant Registry in another state with a minimum of 8 hours of work in that state within the last 24 months may apply for reciprocity. They must:

  • Complete the Interstate Endorsement Forms
  • Attach a copy of their social security card, nurse aide certification and all other items listed in the Employment Verification section
  • Include test results from their Nurse Aide competency exam if they took the test within the last 24 months
  • Scan completed application and all requested items and send to [email protected] in one email

Reciprocity applicants aren’t eligible to work in Minnesota nursing homes or certified boarding care homes until their request has been approved and their name added to the Minnesota Nurse Aide Registry. It can take up to 30 days to process endorsement applications.

Missouri

CNA Certification Requirements

Missouri contracts with D&S Diversified Technologies/Headmaster for CNA testing and scoring. Applicants meeting all the following requirements will be listed on the Missouri Certified Nurse Aide Registry (MOCNAR).

  • Complete a state-approved CNA course comprised of 75 hours of classroom training and 100 hours of on-the-job training
  • Meet all federal and/or state requirements
  • Pass the two-part (written/oral knowledge and skills) competency exam within 1 year of the training start date
    • $30 knowledge exam
    • $40 audio version of the knowledge exam
    • $95 skills test

Candidates who don’t pass both parts of the exam may attempt one or both parts twice more, within 90 days of the initial exam. Upon failing 3 times, they must complete the entire basic course again before retesting.

Applicants who meet the CNA challenge requirement may take the CNA written and practicum exam without taking the CNA course. If they fail either part of the exam, they must complete the full CNA course and retake the exam upon completion.

Renewal Requirements: $20 Fee

  • Must renew electronically every 24 months
  • Must work for pay as a CNA performing nursing or nursing-related services at least 8 consecutive hours during the past 24 months

Reciprocity: No Fee

CNAs certified in another state in good standing can apply for reciprocity in Missouri without taking the Missouri CNA exam. Applicants must:

Nebraska

CNA Certification Requirements

  • Must be at least 16 years old
  • Not have been convicted of a crime involving moral turpitude related to the nurse aide position
  • Able to speak and understand English or the language understood by a substantial portion of the nursing home residents
  • Complete a state-approved training program consisting of at least 75 hours or a 21-hour basic resident-care course for intermediate care facilities for the developmentally disabled
  • Complete one hour of Nebraska-specific abuse/neglect/misappropriation training
  • Pass the Nebraska-approved written/oral exam and skills competency exam

CNA candidates don’t fill out an application or pay a fee. Once they’ve completed all requirements, the facility employer must complete and submit the Registry form to put the CNA’s name on the state’s registry upon hire.

Must work as a nurse aide in a paid position within 24 months of the last date of previous CNA employment.

Reciprocity:  $25-$30 for Online Abuse Training

To qualify for reciprocity, Nebraska requires applicants to:

New Hampshire

CNA Certification Requirements

CNAs are called licensed nursing assistants (LNAs) in New Hampshire. The New Hampshire Office of Professional Licensure and Certification (OPLC) handles LNA licensing.

CNA Certification Requirements

The Oklahoma State Department of Health approves training and competency evaluation programs, including educational-based and employer-based programs. Employer-based programs must complete the State-required criminal background check. All programs must use a performance record/skills performance checklist. Only Department-approved entities approved as test sites with Prometric shall administer and evaluate competency exams, including a written or oral portion in English. 

The Health Certification Project offers additional written certification exams for nurse aides and Oklahoma recognizes each of these roles as a type of CNA:

Utah

CNA Certification Requirements

The Utah Nursing Assistant Registry (UNAR) provides certification and reciprocity services for CNAs in the State. UNAR also approves and oversees all nursing assistant training programs. The CNA certification process in Utah requires applicants to:

  • Complete a 100-hour State-approved CNA training program
  • Must learn and demonstrate the ability to perform over 70 skills documented through the UNAR Nursing Assistant Skill Proficiency List
  • UNAR partners with various assessment centers statewide that conduct nursing assistant certification testing
  • Pass the State’s CNA competency exams, including a knowledge exam consisting of 75 multiple-choice questions and a skills demonstration exam, within 3 attempts and 1 year of program completion
    • $35 written knowledge exam (per attempt)
    • $55 manual skills exam (per attempt)
    • $90 expired certification testing fee

Renewal Requirements: $20 late renewal fee (up to 30 days)/$40 late renewal fee (2-6 months)

  • Must complete at least 200 paid hours as a nurse aide
  • Log in to your TMU account to complete the online Renewal Form
    • Available 90 days prior to expiration and up to 6 months past expiration

Reciprocity: $50 Fee

Vermont

CNA Certification Requirements

The Vermont Board of Nursing uses the licensed nursing assistants (LPN) title and contracts with Excel Testing for nursing assistant competency exams in Vermont.

  • Complete a 75-hour State-approved nursing assistant program
  • Use the Online Services Platform to complete an online application and pay fees
  • Complete a Criminal Background Check
  • Register with Excel Testing for NNAAP knowledge and skills tests

LPN applicants in Vermont may receive provisional licensure for up to 90 days while awaiting their criminal background checks.

  • $25 nonrefundable application fee to VT BON
  • $100 written knowledge test fee to Excel
  • $100 skills test fee to Excel

Renewal Requirements: $65 Fee

LPNs in Vermont must renew their certification every 2 years on or before November 30 of even numbered years.

  • Provide proof of meeting active practice hour requirement of 50 days (400 hours) in the last 2 years

CNAs can fast-track their endorsement if they can show proof of continuous licensure for 3 years. Applicants may receive provisional licensure for up to 90 days if all their application is missing is the criminal background check or licensure verification.

CNA Certification Requirements

CNAs who don’t meet employment requirements must take and pass the nurse aide state exam again before applying for renewal of certification.

Applicants for certification by endorsement must submit written evidence that they:

  • Possess certification in good standing to practice as a CNA from another State or U.S. territory with requirements similar to VBON’s requirements
  • Haven’t committed any act or omission constituting grounds for discipline or denial of certification
  • Have no record of negligence, abuse or misappropriation of property or any disciplinary action taken/pending in any other State or U.S. territory

Washington

CNA Certification Requirements

The Washington Board of Nursing (WABON) is responsible for the state certification exam for nursing assistants. The Washington State Department of Health (DOH) issues nursing assistant certifications, endorsements, renewals and the rules governing certification.

  • Complete a minimum of 85 hours of training through a State-approved program, including 7 hours of AIDS/HIV training
  • Get a Credentia Candidate ID number (required to register for testing)
  • Register for the skills evaluation exam first through the CNA training program or WABON online scheduling if training program doesn’t offer testing
  • Pass the skills evaluation exam before registering for and passing the written nursing assistant competency exam administered through Credentia
  • Apply for nursing assistant-certified (NAC) credential with the Department of Health

Application Fees

  • $85 application fee
  • $55 written/oral exam fee
  • $100 skills evaluation fee

Renewal Requirements: $95 Certification Renewal Fee / $10 Endorsement Renewal Fee

CNAs must renew their certification on or before their birthday each year. CNAs who renew late must pay a $50 late fee along with the renewal fee. There aren’t any continuing education requirements.

Reciprocity: $25 Fee

Applicants applying for endorsement from another State must:

  • Provide verification of placement on a State OBRA and current CNA certification
  • Provide proof of completion of training requirements
  • Have exam results sent directly to the Department of Health
  • Submit work history, including professional training and experience
  • List all states where currently hold or previously held credentials, including those where applicant applied for a credential but wasn’t granted
    • Jurisdictions where applicant is/was credentialed must complete verification forms and send them directly to the Department

  • Have employers (except nursing homes) verify employment

Reciprocity: No Fee

CNAs listed on other States’ registries in good standing where the other States’ requirements are substantially the same as West Virginia’s education and competency evaluation requirements may apply for inclusion on the WV Nurse Aide Registry by endorsement. 

Applicants must complete a Nurse Aide Application for Reciprocity with the required attachment(s) and submit it to the Office of Health Facility Licensure and Certification Nurse Aide Program within the West Virginia Department of Health and Human Resources for review.

Wisconsin

CNA Certification Requirements

Wisconsin utilizes D&S Diversified Technologies/Headmaster to provide nurse aide competency testing and scoring services and maintenance of the Wisconsin Nurse Aide Registry (WNAR). 

Candidates must:

  • Complete an approved 75-hour Wisconsin Nurse Aide training program
  • Pass the competency evaluation consisting of a multiple-choice knowledge exam and a skills test
    • $125 for Written Knowledge and Demonstration/Skills exams
    • $130 for Oral Knowledge and Demonstration/Skills exams
    • $32 written knowledge exam only
    • $37 oral knowledge exam only
    • $93 demonstration/skills exam only
  • Meet all other requirements of the Wisconsin Department of Health Services to be listed on the WNAR

Renewal Requirements: Fee Unlikely

  • Must renew every 24 months, beginning up to 90 days before expiration of current certification
  • Work for pay as nurse aide for at least 8 hours under RN/LPN supervision during the past 24 months
  • Log in to the online portal and list work hours and where worked to allow an email verification link to be sent to employer for employment verification
    • Applicants without required work experience must take and pass both components of the competency exam again

Reciprocity: No Fee

  • Be current and in good standing on a nurse aide registry in another state
  • Complete the Nurse Aide Registry Out-of-State Applicant form
  • Attach a copy of the certificate/diploma received for completion of a basic nurse aide course or a transcript verifying completion of a basic nurse aide course
  • Must include date of completion and number of training hours received
  • Alabama applicants who originally trained in Alabama more than 2 years ago and are only certified in Alabama must also include employment verification

Applicants from California, Colorado, District of Columbia, Florida, Louisiana, Mississippi, Missouri, North Carolina or Pennsylvania must complete part 1 of the application and submit it and all supporting documentation to:

Wyoming

CNA Certification Requirements

Applicants must create a profile in Wyoming’s nurse portal before submitting their CNA application. Wyoming State Board of Nursing (WSBN) no longer accepts paper applications; all payments must be made by debit or credit card. WSBN contracts with D&S Diversified Technologies/Headmaster to administer CNA exams. Candidates must:

  • Complete a state-approved CNA training program
  • Send application and fingerprint cards to the WSBN
  • Complete a criminal background check from the Department of Criminal Investigations and the FBI
  • Pass the CNA knowledge and skills tests

Wyoming offers a one-time temporary permit to CNAs for 120 days for a $25 fee.

Renewal Requirements: $50

  • Work 400 hours in a CNA role
  • Work 200 hours in a CNA role and complete 15 hours of continuing education
  • Complete 30 hours of continuing education
  • Pass the NATCEP

Applicants who haven’t worked 400 hours or 200 hours and completed 15 hours of CE in the past 5 years must complete a state-approved CNA training and competency evaluation program again or pass the NATCEP or similar national exam again.

Reciprocity: $60 without Temporary Permit / $85 with Temporary Permit

  • Create an account on the WSBN’s Nurse Portal
  • Apply for licensure by endorsement
  • Hold current CNA certification in good standing
  • Never been certified in Wyoming
  • Submit fingerprint cards for a Criminal Background Check from the Department of Criminal Investigations and the FBI

Endorsement applicants may request a temporary permit on their application, usually issued within 1 to 3 business days after receipt of completed application and all required documentation.
