Description
American Traveler is seeking a Physical Therapist (PT) for a home health assignment in Solano County, CA, requiring a current CA PT license and a minimum of 1 year of home health experience.
Details
- Home health setting serving Solano County, CA, with the potential to expand territory as needed
- Schedule is Saturday through Wednesday, 40 hours per week
- Day shift
- Caseloads and visit mixes are structured to average 8 hours per day and 40 hours per week
- Productivity requirement of 30 points per week, with a point-to-hour conversion of 1 point = 1.33 hours
- Visit types include Start of Care (2 pts), Recertification/Resumption of Care/OASIS Discharge/Evaluation (1.5 pts), and Revisit/Non-OASIS Discharge (1 pt)
- Documentation system: Kinnser
- iPad and Verizon One Talk device issued for field use
- 13-week contract
Requirements
- Current, active California PT license required
- Minimum 1 year of home health experience required
- DOB and SSN may be required for consideration
Additional Information
- Responsibilities include prioritizing start of care visits and supervising Physical Therapy Assistants (PTAs)
- Next-day mandatory reporting of start of care visits is required
- Clinical notes and all other documentation must be completed within 48 hours of each visit
- Timely communication with supervisor, intake, and office staff is expected, with general responsiveness during normal business hours
- Orientation consists of 3 full days: 1 day in-house and 2 days in the field
- Mileage reimbursement provided from first to last patient visit; parking and toll costs are also reimbursed
- Overtime must be pre-approved by the Clinical Supervisor
