Description
American Traveler is seeking an experienced RN Case Manager with at least 5 years of acute care experience and a valid CA RN license for a hospital-based inpatient case management role requiring cross-unit flexibility and strong discharge planning expertise.
Details
- Inpatient acute care hospital setting with cross-unit coverage across various departments as needed
- Strong focus on hospital-based discharge planning to the community, not utilization management
- Emergency Department case management experience is highly valued
- Variable shift start times between 7:00 AM and 3:30 PM, with day (7:00–15:30) and evening (15:00–23:30) shifts available
- 5x8-hour shifts
- Every other weekend required
- Floating to various units as needed
Requirements
- Active CA RN license required
- Current BLS certification (AHA or ARC only)
- CCM or ACM certification required
- Minimum 5 years of nursing experience
- Direct hospital-based inpatient discharge planning experience required
- Experience working independently in an acute care hospital setting
- Working knowledge of California insurance requirements and county regulations related to complex discharges
- 2 supervisor references obtained within the past year required for consideration
Additional Information
- Manage complex inpatient discharges with a focus on community-based care transitions
- Must be cross-trained and available to work across all units as needed
- Local candidates are accepted; candidates must reside beyond a 50-mile radius to qualify for travel rates
- Previous travelers or staff who have separated may return without a mandatory waiting period; former manager's name will be required
- Up to 7 days of RTO may be requested per contract; only 1 holiday per contract may be requested off
- Holidays with coverage expectations include: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day (holiday coverage window begins at 7:00 PM the night before and ends at 7:00 AM the day after)
