Job Opportunity: RN Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)

Job Summary

  • Provide home health nursing care including patient assessment, care plan implementation, and ongoing education/support for patients and families.
  • Perform admission processes and case management in accordance with organizational policies and community health standards.
  • Deliver patient care consistent with organizational philosophy and standards.
  • Utilize thorough and timely electronic documentation and interdisciplinary communication.
  • Ability to travel to assigned cases.
  • Weekend rotations occur once every 6 weeks.

Required Experience

  • Minimum 1 year of Certified Home Care experience.
  • Minimum 1 year of Case Management experience.

Contract Length

  • Approximately 13 weeks.

Credential and Compliance Requirements

Licensure & Professional Credentials

  • Valid nursing license with primary source verification (NURSYS and Board of Nursing required).
  • No Massachusetts Conditional Licenses accepted.
  • Resume including past 7 years of work experience with city and state details.
  • Two recent professional references (minimum one supervisor or above).
  • Skills checklist specific to the role.

Background & Screening

  • Criminal background check within 45 days prior to start, including county and international searches if applicable.
  • Massachusetts CORI (criminal history) check within 30 days prior to start.
  • Social Security Number trace covering last 7 years.
  • National sex offender registry check within 30 days prior to start and updated annually.
  • Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days prior to start and annually.
  • Motor Vehicle Record (MVR) check within 45 days prior to start (required for all home care positions).

Health & Immunization Requirements

  • Fit test for N95 respirator within 365 days of start date, updated annually.
  • Documentation of TB screening (skin test or blood test) within 3 months of start date, updated if >6 month break.
  • Health statement signed by a licensed provider within 365 days of start date, updated if >6 month break.
  • Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions allowed.
  • MMR vaccination or proof of immunity.
  • Hepatitis B vaccination or immunity proof strongly preferred.
  • Tdap vaccine within last 10 years preferred.
  • Respirator Medical Clearance attestation if fit test required.
  • If positive TB test, documentation of negative chest x-ray required.

Additional Notes

  • Must submit a professional badge photo (color, recent, plain background, no glasses).
  • All credentialing and compliance documents must be current and uploaded as required prior to onboarding.
  • No religious or medical exemptions for flu vaccine or fit testing.
  • Travel to patient homes is required as part of the role.

This position is ideal for experienced home health RNs with case management skills who are comfortable working independently in community settings and meeting rigorous credentialing and health compliance standards.
