Description

Job Opportunity: Registered Nurse - Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (approximately 13 weeks)


Job Summary

Provide clinical nursing services to patients of all ages in their homes. Responsibilities include assessing patient conditions, implementing care plans, and offering ongoing education and support to patients and their families. Perform admissions and case management duties following established policies and standards, with thorough electronic documentation and interdisciplinary communication.

Weekend rotations occur approximately once every 6 weeks. Must be willing to work weekends, holidays, and be on-call as needed.


Required Qualifications

  • Valid Registered Nurse (RN) license in Massachusetts
  • Minimum 1 year of Home Health experience within the last year
  • Minimum 1 year of Case Management experience
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Experience in a Home Care setting
  • Reliable, independent transportation for home visits
  • Willingness to work weekends, holidays, and participate in on-call rotations

Required Credentials and Compliance

  • Professional license verification (primary source) within 30 days of start
  • Submission of a recent professional photo (color, plain background, no filters)
  • Criminal background checks including state and national registries, updated as required
  • Motor vehicle record check within 45 days of start
  • Documentation of immunizations and health screenings:
    • MMR (measles, mumps, rubella) vaccination or immunity proof
    • TB test or blood test within 3 months of start, updated if break >6 months
    • Health statement from a licensed provider within 1 year of start
    • Fit test for N95 respirator within 1 year of start, updated annually
  • Skills checklist completed within 1 year of submission
  • Resume including 7 years of work history with city and state
  • Two professional references within 4 years, including at least one supervisor-level
  • Education verification of highest degree achieved
  • Additional background checks including CORI, SSN trace, exclusion lists, and nurse aide registry as required

Additional Information

  • Must be able to work independently in patient homes
  • Strong documentation and communication skills required
  • Compliance with all health and safety regulations, including respirator medical clearance if applicable
