RN Case Manager Travel Assignment

Location: White Plains, New York
Contract Length: 13 weeks

Job Summary

• Perform initial comprehensive assessments on admission following Care Management policies.
• Screen patients for high-risk indicators to ensure appropriate supportive services for discharge and prevent readmission.
• Provide ongoing reassessment of patient needs throughout the hospital stay.
• Conduct utilization review (UR) activities per department policy.
• Coordinate multidisciplinary care within and between hospitals.
• Manage transitional planning including patient/caregiver education, aftercare arrangements, commercial payer involvement, and addressing high utilizer patient root causes.
• Collaborate and communicate relevant patient information with healthcare team members.
• Support department goals such as length of stay (LOS) reduction, readmission prevention, denial reduction, and patient satisfaction improvement.
• Participate in performance improvement activities.
• Collaborate on safe discharge planning for geriatric patients across inpatient, emergency, and surgical outpatient settings.

Required Qualifications

• Valid Registered Nurse (RN) license in New York State.
• Minimum 2 years of case management experience, including experience within the last year.
• Basic Life Support (BLS) certification issued by the American Heart Association (AHA).
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Experience in utilization review, specifically with Milliman/InterQual criteria.
