Home Health Registered Nurse
Job Overview
- Location: Salida, California
- Employment type: Travel/Contract
- Shift: Day, 5x8 hours, 8:00 AM–5:00 PM
- Hours per week: 40
- Contract length: 13 weeks
Role Summary
Provides in-home nursing care, including assessment, care coordination, direct patient care, patient/family education, and transitions of care. The role also involves developing and managing an interdisciplinary plan of care and maintaining communication with patients, families, physicians, and other healthcare providers.
Required Qualifications
- Valid RN license in California
- Home Health (1 year of experience)
- Home Health experience within the last year
- Associate of Science in Nursing / Associate Degree in Nursing
Work Location / Coverage
- May float within Stanislaus County, San Joaquin County, and Merced County
