Job Opportunity: RN Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)

Job Summary

  • Provide home health nursing care including patient assessment, care plan implementation, and ongoing education/support for patients and families.
  • Perform admission processes and case management in accordance with organizational policies and community health standards.
  • Deliver patient care consistent with organizational philosophy and standards.
  • Utilize thorough and timely electronic documentation and interdisciplinary communication.
  • Ability to travel to assigned cases.
  • Weekend rotations occur approximately once every 6 weeks.

Experience Requirements

  • Minimum 1 year Certified Home Care experience.
  • Minimum 1 year Case Management experience.

Contract Length

  • Approximately 13 weeks.

Credential and Compliance Requirements

Required for Submission:

  • Professional license/certificate verification (Primary source verification within 30 days of start; MA Conditional Licenses not accepted).
  • Resume including past 7 years of work experience with city and state details.
  • Two recent professional references (minimum one supervisor or above).
  • Skills checklist.
  • Candidate submission coversheet.

Required for Onboarding:

  • Color photo (taken within past 6 months; no selfies, filters, or eyeglasses; plain background).
  • Motor Vehicle Record (MVR) check within 45 days of start.
  • N95 Fit Test within 365 days of start and updated annually (no religious exemptions).
  • Nurse Aide Registry check within 30 days of start (required by MA DPH for all employees).
  • Documentation of MMR vaccination or proof of immunity.
  • Influenza vaccine during flu season (Sept 15 - April 30), no exemptions allowed.
  • TB test documentation within 3 months of start (skin test or blood test) and updated if >6 month break.
  • Physician health statement within 365 days of start and updated if >6 month break.
  • Criminal background check within 45 days prior to start and updated if >6 month break.
  • Massachusetts CORI background check within 30 days prior to start and updated if >6 month break.
  • Confirmation of assignment/POI form upon acceptance and for extensions.
  • Respirator Medical Clearance attestation (part of fit test record).
  • Education verification through background check company.
  • Social Security Number trace with criminal check.
  • National sex offender registry check within 30 days of start and updated annually.
  • Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually.

Optional but Preferred:

  • Hepatitis B vaccination or proof of immunity or declination.
  • Tdap vaccination within 10 years.
  • Documentation of chest x-ray if positive TB test.
  • TB symptom survey if positive PPD (updated annually).

Additional Notes

  • No religious or medical exemptions allowed for influenza vaccine or N95 fit testing.
  • Fit testing cannot be performed onsite except in limited island locations if assignment is extended.
  • All documentation must be current and compliant with Massachusetts Department of Public Health regulations.

This position is ideal for experienced home health RNs with case management skills who are comfortable traveling to patient homes and working within a structured, policy-driven environment.
