    Uniform / Scrub Colors

    • RNs: Royal Blue or Navy Blue
    • Pediatric RNs: Pediatric-friendly top with Royal or Navy Blue bottoms
    • RRT: Hunter Green
    • OR/Endo/L&D/PACU: Provided
    • Phlebotomy: Navy Blue (any color acceptable)
    • Imaging Tech: Black

    Required Certifications

    • ACLS (AHA or Canadian Heart & Stroke Foundation)
    • BLS (AHA or Canadian Heart & Stroke Foundation)
    • PALS (AHA or Canadian Heart & Stroke Foundation)
    • NRP (AAP, AHA, or Canadian Paediatric Society)
    • Chemotherapy (if applicable)
    • Electronic Fetal Monitoring (AWHONN or NCC-EFM, if applicable)
    • IV Certification (if applicable)
    • Management of Assaultive Behavior (MAB) (if applicable)
    • NIH Stroke Certification (every 2 years)
    • CPI (Crisis Prevention Institute) (if applicable)
    • State License (must be current and valid)
    • NURSYS Verification (for all RNs)

    Required Background Checks & Verifications

    • Background Check within 45 days of start, covering all locations lived/worked in past 7 years
    • EPLS/GSA/SAM (updated monthly)
    • HHS/OIG (updated monthly)
    • Education Verification (3rd party verification required)
    • State-specific searches as applicable (e.g., Washington WSP/WATCH)
    • Oregon Abuse/Neglect Search (if applicable)
    • WA Nurse Aide OBRA Registry Search (for CNAs in Washington at Skilled Nursing Facilities)

    Health & Immunization Requirements

    • Physical Exam within 1 year of start (signed by MD, NP, PA, or DO)
    • Fit Mask test (annual, travel only)
    • Tetanus/Diphtheria/Tdap (within 10 years, declination accepted)
    • Negative TB test or Chest X-Ray within 12 months (annual update required)
    • MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer, declination accepted
    • Hepatitis B vaccination or signed declination
    • Varicella - 2 vaccines or positive titer, declination accepted
    • 10 Panel Drug Screen within 90 days of start (no instant tests)
    • Flu Vaccine during flu season (declination accepted)
    • COVID-19 Vaccine or declination (required if onsite)
    • Respiratory Safety Clearance (annual for travel)

    Competency Requirements

    • Medication Test/Pharmacology Competency Exam (as applicable)
    • Color Blind Testing (if required by unit)
    • Competency Exams as applicable by specialty
    • Fetal Heart Rate Monitoring Assessment or valid EFM certification for L&D RNs

    Documentation & History

    • Minimum 2 references covering 1 year of employment within last 3 years (supervisor references)
    • Employment Application covering at least last 5 years (7 years for APN/Subcontractors)
    • Skills Checklist (updated annually, as applicable)
    • Radiation Badge (facility issued if applicable)
    • Clarification for any employment gaps or overlaps

    Facility Specific Requirements

    • Floating Policy Acknowledgement (may be required to float to nearby Providence facilities)
    • Badge Photo (passport style with neutral background)
    • Completion of Providence WorkRamp Modules (Core, Specialty, Regional, and Annual Regulatory modules) prior to start
    • Epic Access Training Modules (completion required for Providence system)
    • Providence System TB Questionnaire (annual)
    • Kronos Job-Aide & Attestation Form (if applicable)

    Notes

    • Travelers must include Return to Office (RTO) requests at time of submission (max 2 requests)
    • Background checks and verifications have specific timelines and renewal requirements
